Best Practice C) Patient Review and Coordination (PRC) Information
Goal: Ensure hospitals know when they are treating a PRC patient and treat accordingly
PRC clients are frequent ED users, defined as having five or more visits in the past 12 months. They can often also be narcotic seekers.
How to Accomplish
Hospitals and ED physicians need to know who in their organization will receive the PRC client list from the Health Care Authority and use it to identify these patients on arrival. Each of these patients should have a care management plan. Organizations experience the best success with these patients when utilizing case management within the ED.
